All coaches, assistant, and team moms MUST fill out a NEW Little League Volunteer Application -
they are NOT transferable from year to year. If one is not filled out
the person will NOT be allowed on the field. There will not be any
exceptions to this rule! To print out a copy of the form please click
on the file below and send it in with a copy of your drivers license to DDLL, Attention: Safety Officer, P.O.Box 1075, Dingmans Ferry, PA 18328
